Claiming your Affordable Connectivity Program (ACP) Credits
The information provided below will be used to verify eligibility for the Affordable Connectivity Program (ACP). In order to claim credits, your application must be approved by USAC/National Verifier. Please wait until you received a response of “QUALIFIED” before starting the claim process below.
Ensure you have received an approval status of “QUALIFIED” from the https://acpbenefit.org (USAC National Verifier) site after you entered your information to verify eligibility.
Fill out the Kinetic by Windstream ACP Information form below with the information that was submitted and approved by https://acpbenefit.org (USAC National Verifier). This information must match exactly to prevent delays in having proper credits applied to your Kinetic by Windstream Account. Completion of this form is consent to enroll or transfer the ACP benefit to your Kinetic account.
Pro Tip! Take a screenshot of the information you submitted to https://acpbenefit.org (USAC National Verifier) and use that same copy to fill out this form to ensure an exact match!
Submit your Qualified Application ID and matching information via the Kinetic by Windstream ACP Information form.
Once your submission is processed and credits applied, you will receive an email confirmation from Kinetic by Windstream.
Credits to your Kinetic by Windstream account will appear on your next bill.

Kinetic by Windstream will contact you via phone and/or email if there are any issues when claiming your Affordable Connectivity Program credits.

Terms & Conditions
The Affordable Connectivity Program is a federal government benefit program operated by the Federal Communications Commission that provides a discount on broadband service plans for eligible consumers.
You may get ACP-supported services from any participating provider of your choosing, and you may apply the benefit to any broadband service offering of that provider at the same terms available to households that are not eligible for the ACP.
Windstream may disconnect your ACP-supported service after 90 consecutive days of non-payment. You will be subject to usual undiscounted rates and general terms and conditions if the Affordable Connectivity Program ends, if you transfer your benefit to another provider but continue to receive service from Windstream, or if you de-enroll from the ACP.
You may file a complaint about your ACP-supported service with the FCC at 1-888-225-5322 or at (https://consumercomplaints.fcc.gov/hc/en-us/articles/4412582232980-Need-Help-with-an-Affordable-Connectivity-Program-ACP-or-Emergency-Broadband-Benefit-EBB-complaint)
We will provide you with a list of all ACP-supported service plans that are fully covered by the benefit amount for which your household is eligible.
If transferring, this disclosure is to provide you with notice that you will be transferring your affordable connectivity program benefit to Windstream. Your affordable connectivity program benefit will be applied to the Windstream’s service and will no longer be applied to the service you have from your previous provider that applied your program benefit.
You may be subject to the undiscounted rates of the service of the provider through which you previously obtained program benefits if you elect to maintain service with that provider.
You are limited to one affordable connectivity program benefit transfer transaction per service month, with limited exceptions such as unwanted transfers or where you are unable to receive the requested service.
Your submission indicates that you were provided with these disclosures, and that you give your informed consent to transfer your benefit to Windstream by the next bill cycle.
