The Affordable Connectivity Program is a federal government benefit program operated by the Federal Communications Commission that provides a discount on broadband service plans for eligible consumers.
You may get Lifeline-supported services from any participating provider of your choosing, and you may apply the benefit to any broadband service offering of that provider at the same terms available to households that are not eligible for the Lifeline.
Windstream may disconnect your Lifeline-supported service after 90 consecutive days of non-payment. You will be subject to usual undiscounted rates and general terms and conditions if the Affordable Connectivity Program ends, if you transfer your benefit to another provider but continue to receive service from Windstream, or if you de-enroll from the Lifeline. We will provide you with a list of all Lifeline-supported service plans that are fully covered by the benefit amount for which your household is eligible.
If transferring, this disclosure is to provide you with notice that you will be transferring your lifeline benefit to Windstream. Your lifeline benefit will be applied to the Windstream’s service and will no longer be applied to the service you have from your previous provider that applied your program benefit. You may be subject to the undiscounted rates of the service of the provider through which you previously obtained program benefits if you elect to maintain service with that provider.
You are limited to one lifeline benefit transfer transaction per service month, with limited exceptions such as unwanted transfers or where you are unable to receive the requested service.
Your submission indicates that you were provided with these disclosures, and that you give your informed consent to transfer your benefit to Windstream by the next bill cycle.
